Medical Evaluation
Primary Diagnosis: Bulging discs
cervical spine.
History noted: "restrained driver when involved in a MVA
on 5/5/05
while stopped at a traffic light when he was rear-ended and hit
the car
in front of him. He was thrust forward and backwards.
Patient was seen
in ER in Knoxville, Tennessee. He has been under the care
of Dr. Jack
Bookman and several other doctors. He has had an MRI of
the neck and
back. He has also undergone a myelogram and was then
referred here.
Surgery has been suggested on the cervical spine. Chief
complaint
of neck pain which radiates between his shoulders and is limited
with movement. Complains of headaches since he had the
myelogram."
Current Medications: Paxil, Ambien, Demerol, Parafon
Forte, Diovan
Smoker 1/2 pack per day, just started smoking again.
