Medical Staff -
General
(1)
A copy of all contracts, agreements, protocols, letters, memoranda
or
instruments
in writing that in any way relate to the employment or affiliation
of the ER MD
on duty at the time in question with/by County General
Hospital.
(2) A copy of any and all grievances,
complaints, criticisms, letters of
Condemnation
and recommendations by County General Hospital
regarding the ER MD at
any time up to the present.
(3) A copy or the ER MDs license to
practice medicine in the state of Tennessee
for the year
2005.
(4) A copy of all instruments in writing to
and from the American Medical
Association
physician master file regarding the ER MD and/or a description
of all verbal
communication between a representative or agent of County
General
Hospital and said organization regarding the ER MD.
(5) A copy of all instruments in writing to
and from the National Practitioner
Data Bank
regarding the ER MD and/or a description of all verbal
communication
between a representative or agent of County General
Hospital and said Data Bank
regarding the ER MD.
Nurses
- General
(1) A copy of all
policies and procedures in effect in 2005 which set out or
specify the
scope of conduct of patient care to be rendered by the
Emergency
Department nurses.
(2) A copy of the table of contents for
every policy and procedure manual
located in
the Emergency Department.
(3) A copy of all written material
regarding the definition of the nursing scope
of practice
in the Emergency Department.
(4) A copy of all policies and procedures
regarding nursing orders, nursing
standards of
practice parameters and guidelines, nursing practice policies,
nursing audit
criteria and/or patient outcome criteria for the Emergency
Department.
(5) A copy of the nursing licenses of the
registered nurses who cared for the
patient
during the period in question.
Interfacility Transfer -
General
(1) A copy of all
policies and procedures in effect in 2005 which set out or
specify the
scope of conduct of patient care to be rendered by emergency
medical
technicians (EMTs) and/or paramedics.
(2) A copy of the table of contents for
every policy and procedure manual.
(3) A copy of all policies and procedures
regarding EMT/paramedic orders,
standards of
care, standards of practice, practice parameters and guidelines,
practice
policies, audit criteria and/or patient outcome criteria.
(4) A copy of the EMT and/or paramedic
certification of EMS employees who
cared for the
patient during this period.
(5) A copy of the cardiopulmonary
resuscitation certification of EMS employees
who cared for
the patient during this period.
